Using Data to Drive Dramatic Improvement Summit – 04/16/13

The Summit served as the first face to face Learning & Action Network (LAN) Albany Hospital Referral Region community event to review root cause analysis findings conducted within the community to date, outline best practices within the community, and for representatives across the 17 counties within the project target region to convene to identify opportunities to better coordinate care management of the clinically complex long term care patient population.
