IPRO has been awarded the CMS QIN-QIO 11th SOW expert healthcare quality improvement service contract to continue our quality improvement (QI) work with New York State nursing home (NH) providers to improve healthcare services for Medicare beneficiaries. IPRO, the Quality Innovation Network-Quality Improvement Organization (QIN-QIO) under contract with CMS is charged with supporting nursing home communities to promote improved healthcare services and quality of care for Medicare beneficiaries; working with diverse providers and the community on data-driven quality initiatives to improve resident safety, and improve clinical care; thus, making care safer by reducing Healthcare-Acquired Conditions (HAC) in Nursing Homes.
The IPRO QIN-QIO is developing a strategy to guide local, state and national efforts in improving the quality of care in nursing homes. The IPRO QIN-QIO team of Quality Improvement Specialists is recruiting long-term care community providers to improve nursing home performance through enhanced consumer engagement, creation of strategic partnerships, and the advancement of quality through innovation.
Quality In Action – A Collaborative Approach
Nursing Home Quality Care Collaborative (NHQCC)
The NHQCC is an all teach, all learn initiative, modeled after the Institute for Healthcare Improvement’s Breakthrough Series approach. The initiative is being led by CMS and QIN-QIOs, like IPRO, as part of the 11 Statement of Work (SOW). The Collaborative seeks to rapidly spread the practices of high performing nursing homes to every nursing home in the state, with the aim of ensuring that nursing home residents receive the highest possible quality of care. Specifically, the NHQCC will strive to instill quality and performance improvement practices (QAPI), reduce healthcare-acquired conditions, and improve resident satisfaction by focusing on the systems that impact quality such as: staff stability, communication, leadership, compliance, quality measures and specific clinical outcomes.
- Supports development of strategies for overall quality using QAPI as the framework
- Uses a data-driven and pro-active approach to quality improvement– a measurable aim will drive the work;
- Allows flexibility of the nursing home to choose priority focus areas that are ready for change; and
- Addresses gaps in systems through planned interventions in order to improve the overall quality of care.
The NHQCC Aim:
- Improve Inappropriate Anti-psychotic Medication Use by 15%
- Achieve a Composite Measure score of <6
- Decrease Healthcare-Acquired Conditions
- Decrease potentially Avoidable Hospital Admissions/Re-Admissions
